What to bring
Along with your insurance information, please remember to bring all of your x-ray, MRI and CT films with you to your appointment for a complete assessment of your condition. Also, be sure to fill out your OFAC patient registration and medical history forms (click here to download patient forms) before you arrive to save time. If you can’t do so beforehand we’ll have the forms waiting for you. Please arrive a few minutes early so you can fill out the forms in our office prior to your scheduled appointment time.
Canceling or rescheduling
We understand that sometimes plans can change. If you need to cancel or change an appointment we ask that you give us 24 hours notice. This helps us keep our days organized so we can serve all of our patients as effectively as possible. Like you, occasionally we encounter emergencies that force us to cancel our office appointments on short notice. In this case we will work with you to reschedule your appointment at the earliest convenient time.
What to expect before surgery
If you’re like most people, you may be a little anxious about your surgery. That’s why our skilled staff takes every precaution to ensure the best outcome. Pre-operative testing is performed to determine if you’re able to safely have surgery. We also provide you with specific pre-operative testing and admission instructions so you feel prepared and ready. But above all, we take the time to listen and address your questions and concerns.
Will my family be comfortable if they come to visit?
Our state-of-the-art medical campus ensures that you receive the highest level of care, but it also ensures that you and your family will be comfortable throughout your treatment. We have comfortable waiting areas, Tim Horton’s cafe and child care on site.
