payment & Insurance Information
Co-pays and balance are requested at check-in. We accept personal checks, VISA, MasterCard, American Express, Discover, cash, and most insurance plans. It is extremely important that you bring your insurance cards to your appointment so that we may obtain the necessary and correct information to file all primary and secondary claims for you. If you do not have your insurance card, you will need to reschedule your appointment.

Credit/Debit Card on File Payment Option
Beginning April 1st, 2015, Orthopedic Foot & Ankle Center will provide the option to securely add a debit or credit card to your account for payment of co-pays and other out-of-pocket expenses (deductible, co-insurance, non-covered items, etc.).
With the ever-changing environment in healthcare, in particular the Affordable Care Act and High Deductible Health Plans (HDHPs), more responsibility of payment is being placed on you, the patient. This option will provide an easier, faster and more efficient way of managing the financial aspect of your care. This in no way will compromise your ability to dispute a charge or question your insurance company’s determination of payment.
How do I enroll is this option?
If interested, simply provide us your card information (explained below) and sign a one-page form granting us authorization to charge your card according to the terms of the agreement. All amounts due less than $200 will be automatically charged to your card and a receipt will be sent to you. For amounts due over $200, a billing specialist from our office will contact you before your card is charged so any changes can be made if necessary.
What is the process for collecting my card data?
We prefer that you to fill out the Credit/Debit Card Authorization Form and give us your credit card in person. We will then swipe your credit card with an encrypted reader that will upload your credit card number into our secure gateway and return the card to you. With the encrypted reader, we will never see all the numbers on your credit card. You can deliver your credit card information over the phone or by mail, but the most secure way is in person through the encrypted reader.
How can I trust that you will keep my credit card and checking account information safe?
We do not keep any credit card information or checking account information on file here in the office or on any of the computers here. As mentioned above, we use a secure gateway subject to the PCI DSS guidelines that is completely compliant as required by law.
Can I keep a Health Savings Account (H.S.A.) account or Flexible Spending card on file?
Yes! You can keep your H.S.A. or flexible spending card on file, however, in the event that the funds in your account become insufficient, we will contact you to replace that card information with an active credit or debit card.
